Fifty 410 Tirzepatide Review: Monthly Cost & Refund Stages

Fifty 410 currently lists a $199 monthly compounded tirzepatide program with all dosages included, licensed-provider consultation and refrigerated shipping. Its cheaper-looking $133 monthly headline belongs to a three-month starter program costing $399 total. Those are different purchasing options, not interchangeable monthly bills.³

Monthly purchase versus three-month supply

The Monthly tab advertises a $199 total, month-to-month flexibility and no subscription requirement. The three-month tab lists $399 for the starter program, $499 for the mid-dose program and $549 for the high-dose program. Its $133, $166 and $183 monthly labels are rounded equivalents; use the published totals when comparing the payment due.³

The pricing page says all programs include consultation, medication and free refrigerated shipping, with provider follow-up through the portal. It describes the prescribed program’s medication shipping upfront after payment, rather than monthly shipments from the three-month package. Financing through Afterpay or Affirm has separate eligibility and terms; installment financing does not turn a prepaid supply into a cancelable monthly subscription.³

A clinician selects the regimen. All dosages included describes the monthly price, not permission to choose or change a dose without the prescriber. Confirm the actual quantity, formulation, label instructions and any required testing before payment.

Who provides the care and medication?

The Terms name Aspen Aesthetics, LLC, doing business as Fifty 410 Health, as the platform operator. They distinguish it from the independently licensed clinician who makes treatment decisions and the pharmacy preparing and dispensing medication. The patient must be physically in a state where the evaluating provider is licensed. Starting an intake does not guarantee a prescription.^4,5

The current Terms describe patient-specific compounding under Section 503A. This is the company’s stated pharmacy framework, not independent confirmation of a particular facility’s license or accreditation. Section 503A and Section 503B are not interchangeable credentials, and neither label makes a compound an FDA-approved finished drug. The current program page explicitly says compounded tirzepatide is not FDA-approved and has not been reviewed by FDA for safety, effectiveness or quality. It also says the platform does not compound, manufacture or distribute medication. Promotional review scores or photographs do not establish the effectiveness or quality of an individual prescription.⁴

When payment and cancellation matter

No credit card is required to start. The stated sequence is provider consultation, prescription if appropriate, then selection of pharmacy and supply length through a secure payment link. The refund policy says payment is ordinarily requested only after a provider decides a prescription is appropriate.^3,6

After payment, the policy allows a full refund only when a cancellation is received and confirmed before the dispensing pharmacy begins compounding. Merely sending a request is not confirmation that the cutoff was met. Once compounding begins, routine cancellations and refunds stop; the same applies to routine returns after shipment. This makes the pharmacy’s order status more important than the delivery date.⁶

There are meaningful exceptions. If the platform or pharmacy cannot fulfill an order, the affected unfulfilled amount is refunded. Verified duplicate charges, incorrect orders, loss, non-delivery, damage or compromised medication can lead to the appropriate refund, replacement or credit. Report damaged, incorrect or compromised shipments within 48 hours and retain the requested documentation. For a temperature concern, wait for the dispensing pharmacy’s instructions rather than using a suspect product.⁶

Separately itemized consultations, laboratory work or other services are generally nonrefundable once performed, unless the charge was erroneous or law requires otherwise. Clinical or prescription changes after compounding are reviewed according to their reason, order status and any error. These provisions do not create an unconditional denial-refund or satisfaction guarantee.⁶

Compare the care arrangement with the preparation actually prescribed, not just the lowest monthly equivalent. Judge the provider on price, oversight and disclosure; judge the molecule on its trials. Tirzepatide's are strong — roughly 15–21% of body weight over 72 weeks in SURMOUNT-1¹, and better than semaglutide head-to-head² — but they were run on the brand drug at FDA-approved doses. The practical verdict

The $199 monthly option offers a clearer short-term price comparison than treating a three-month equivalent as a monthly bill.
