Format

Vials and the four-dose KwikPen share the eligible offer ladder; their program eligibility and device handling differ. The single-dose auto-injector has a separate list price.

Dose tier

Used only to look up the self-pay price tier — not a dose recommendation. 2.5 mg is the starting dose; 5 mg is its own tier; 7.5–15 mg share a tier (all doses from 7.5 mg up carry a 45-day refill condition).

Insurance / discount route

The savings card requires commercial insurance and excludes Medicare, Medicaid, the VA and TRICARE. Separate self-pay offers and Medicare pathways have different eligibility rules.
