Willow Tirzepatide Review: Dose Prices, Refills and Refund Terms

Willow advertises its compounded tirzepatide injection at $399 per month, but its detailed pricing article lists higher charges for higher prescribed doses, reaching $549 per month. Compare the dose-specific quote and renewal authorization rather than treating the headline as an all-dose rate.^1,3

The injection price changes with the prescription

The September 2026 help article lists $399 for 2.125 mg, 4.25 mg and 6.375 mg; $499 for 8.5 mg and 11 mg; $524 for 13.5 mg; and $549 for 17 mg. These are provider-published treatment labels, not instructions to select or increase a dose. Willow says pharmacy doses may vary slightly and the doctor determines treatment.³

The table is useful for budgeting, but does not identify your supplied concentration, vial quantity or exact renewal interval. Request those details and the amount due now. Do not map a compounded dose label directly onto a branded pen, stretch a prescription to lower an apparent monthly cost, or infer a prepaid invoice from a monthly advertisement.

The same help article lists compounded tirzepatide tablets at $399 for 6 mg or 10 mg and $459 for 20 mg. Those are separate oral-dissolving preparations, not the injection or an FDA-approved oral tirzepatide product. A shared ingredient does not establish equivalent absorption or clinical results.³

What cancels the next order?

Willow’s help center says there is no long-term commitment and subscriptions can be canceled by messaging. Its refill article sets a more practical boundary: contact Patient Care before the pharmacy processes the prescription. If an already-charged refill is successfully canceled, it describes five to ten business days for the bank to post the refund.^5,6

The linked consent, labeled effective November 2021, instead requires two full calendar days before the next shipping date for subscription cancellation. It separately describes a full refund for cancellation within 24 hours of order placement, while excluding shipped orders. These are different timing descriptions; obtain the cutoff and effective date for your actual prescription order rather than relying on the most generous window.⁴

The August 2026 Terms say submitted orders may be too advanced to cancel, incorporate more specific policies and generally exclude refunds once charges are incurred. They also expressly provide that when a required prescription is not issued after consultation, Willow declines the order and returns payment by the original method. That denial provision does not create a general refund for medication already prepared or a change of mind.²

The separate shop’s Subscribe & Save policy mainly concerns supplements, with approximately 90-day deliveries. It expressly allows different prescription subscription terms. Do not import its billing frequency or cancellation calculation into tirzepatide care. Likewise, the shop’s final-sale policy and 14-day damaged-item claim process do not by themselves establish the remedy for a particular prescription shipment.^10,11

Keep confirmation of a cancellation and ask separately about future renewal, an existing charge and a damaged shipment. A successfully stopped subscription does not necessarily reverse an order already in pharmacy processing.

Insurance and reimbursement descriptions conflict

Willow’s insurance help article suggests possible private-plan reimbursement by submitting receipts. The current Terms instead say neither the platform nor partners submit third-party claims and the patient agrees not to submit reimbursement claims to third-party payors. Confirm the applicable payment agreement and insurer’s position before relying on reimbursement; the public pages do not resolve that conflict.^2,7

HSA/FSA eligibility is advertised, but it is not the same as an insurance claim or a guarantee of plan reimbursement. Ask your account administrator about the documentation required for your selected purchase.³

Clinical support and pharmacy assignment

The product page advertises unlimited doctor messaging and 24/7 Care Team support. Its shipping help describes a clinician review, prescription processing, pharmacy preparation and tracked delivery, with most deliveries within seven days of approval and possible delays. Free two-day shipping is a shipping description, not a two-day guarantee from application to delivery or a guaranteed clinical response time.^1,8

Willow Health Services, Inc. is the platform, while the linked consent names Willow Medical of California, P.C. for clinical consultations. Its physicians page names Michael Green as Chief Medical Officer and lists other clinicians. These are provider disclosures; confirm the actual clinician and the scope of follow-up available to you.^4,9

The Terms name Red Rock Home Pharmacy, Epiq Pharmacy, Absolute Pharmacy, The Pharmacy Hub, Empower Pharmacy and a Red Rock St. George location. That is a potential partner list, not confirmation of your dispenser, its license or the testing for your own vial. Obtain the pharmacy’s legal name, preparation, storage instructions and beyond-use date before use.²

The consent contains two state lists that differ on Wyoming, while the current Terms require the physician, practice and pharmacy to be authorized where you are physically located. Ask for confirmation for your location rather than assuming nationwide availability or resolving the inconsistent lists yourself.^2,4

Our assessment

Willow provides a specific injection headline, a more informative dose-price schedule and described physician messaging. The main limitations are the higher-dose cost and overlapping public cancellation and reimbursement descriptions. Get the selected prescription, complete charge and cancellation terms together before accepting renewal.
