Search for what GoodRx charges for Zepbound and you will land on $299 a month. That number is real, it is current, and we confirmed it live. It is also not a GoodRx discount — it is Eli Lilly's own self-pay price, displayed on GoodRx's page under a manufacturer badge, sponsored by Lilly and governed by Lilly's terms. GoodRx's actual negotiated coupon for Zepbound is a different, much larger number, and it sits one tab away.

That distinction is the whole story of this page, and it changes what you should do with the quote. What GoodRx quoted for the Zepbound KwikPen

GoodRx's default Zepbound view is the KwikPen — four doses in one 2.4 mL pen, which is a 28-day supply. Every price below is badged "MANUFACTURER OFFER" and labeled "per month, price without insurance"¹:

Those are the four strengths we priced individually. GoodRx's own fine print groups 7.5 mg, 10 mg, 12.5 mg and 15 mg into a single pricing tier, so the two we did not open separately sit in the same band as the two we did.

Why that ladder should look familiar

It is Lilly's ladder. LillyDirect's Self Pay pricing, in effect since December 1, 2025, is $299 at 2.5 mg, $399 at 5 mg and $449 from 7.5 mg up for a 28-day supply². GoodRx's numbers match at every tier we checked — not approximately, exactly.

One detail is worth stating precisely, because the press release that announced this ladder is titled for vials while GoodRx quotes it against the KwikPen. Lilly's own savings page resolves it: "The $299 per month is for the Zepbound KwikPen or vial, not Zepbound single-dose Pen"⁵. The offer spans two of the three formats. It is the single-dose Pen that falls outside it — which is exactly the format GoodRx's own $995 coupon applies to, and the reason the two numbers on that page are so far apart.

Three further details confirm this is a pass-through rather than a coincidence. The offer is flat across every pharmacy: at ZIP 75270, CVS, Walgreens, Walmart, Kroger, Costco, Sam's Club, Target, Albertsons, HEB and the rest all returned the identical $299.00¹. It is also flat across ZIP codes — re-running the same item at ZIP 10001 in New York returned $299.00 again. A price genuinely negotiated pharmacy by pharmacy does not behave that way; a manufacturer price does. And the small print names its owner outright: *"Card eligibility and terms and conditions may be terminated, rescinded, revoked, or amended by Lilly at any time without notice and for any reason. PROGRAM IS NOT INSURANCE,"* with full terms hosted at zepbound.lilly.com¹.

So the practical reading is this: GoodRx is showing you Lilly's price and letting you redeem it at a retail counter. That is a genuine convenience — you get the LillyDirect rate without waiting on a mail shipment — but it is not a saving GoodRx created, and it is not a reason to choose GoodRx over LillyDirect on price. The two are the same number.

The 45-day condition rides along with it

Because it is Lilly's program, it carries Lilly's condition, and GoodRx reproduces it. From 7.5 mg upward the page reads: *"For 7.5 mg, 10 mg, 12.5 mg and 15 mg doses, prices reflect monthly cost with Zepbound Journey Program. Regular prices will apply if prescription is not refilled within 45 days."*¹

That is the same 45-day refill window attached to LillyDirect self-pay, and letting it lapse is expensive — the 15 mg dose reverts to roughly $1,049 a month². The $449 is a price for someone refilling on schedule, not a standing rate. The rest of the terms are worth reading in one piece: self-pay only, eligibility required, savings subject to monthly and annual limits, and a "month" defined as 28 days and one single-patient-use KwikPen¹. Taxes and fees may apply.

GoodRx's own coupon: $995

Switch the format from the KwikPen to the single-dose pen — sold as a carton of four prefilled pens, also a 28-day supply — and the manufacturer offer disappears. What replaces it is labeled "Standard GoodRx coupon — free to use, no insurance required", and this one is genuinely GoodRx's. At ZIP 75270, for the 2.5 mg carton¹:

This behaves the way a negotiated cash price should: it varies by chain, it prints a struck-through retail figure beside it, and it does save real money against that figure — about $286 off at CVS and $310 off at Walmart. It is also $696 a month more than the $299 KwikPen route for the same drug at the same 2.5 mg dose. GoodRx flags the gap itself, with a "Switch to Zepbound KwikPen for $299" button sitting directly above the coupon list.

The lesson is not that GoodRx is hiding anything — it is not; the switch prompt is right there. The lesson is that the format your prescription is written for decides your price, and the difference is larger than any discount card can close. If your prescriber wrote you single-dose pens, the most valuable ten seconds of this whole exercise is asking whether the KwikPen is appropriate for you instead.

If you have commercial insurance, neither number applies

GoodRx's single-dose pen view also carries a savings-card tab reading "Pay as low as $25" for a 1- or 3-month supply, badged "Requires private insurance" and conditioned on being "eligible and commercially insured with coverage for Zepbound"¹. That is Lilly's manufacturer savings card, and it matches the terms Lilly publishes directly³.

If your commercial plan covers Zepbound, that $25 tier beats every cash price on this page by an order of magnitude, subject to an annual savings cap. If your commercial plan excludes Zepbound, the card's non-covered tier puts the single-dose pen at $499 a month³ — still half the $995 coupon. The honest verdict

For brand Zepbound, GoodRx is worth using — but for the reason on the label rather than the one implied. It shows the cheapest broadly available cash price for the drug, $299 to $449 depending on dose, and lets you fill at a pharmacy near you today instead of waiting on mail order. What it does not do is beat Lilly, because on this drug the price it shows is Lilly's, sponsored and controlled by Lilly, revocable by Lilly, and carrying Lilly's 45-day refill condition.
