Add the membership, medication and testing

The terms describe website medication prices as monthly pricing based on standard use or a starting dose. For the displayed monthly membership, three $129 membership payments total $387; adding the advertised $99 consultation yields $486 before medication, labs, shipping and other applicable charges. This is arithmetic for that advertised membership, not a verified selected patient's invoice.^3,4,5

The same page also presents a $1,399 annual Performance membership with separate medication costs and notes that California prices may vary. Its annual card mentions a free essential lab panel or a discounted comprehensive panel, while the general FAQ says labs are not included. Confirm the selected plan's lab allowance rather than applying an annual benefit to a monthly membership.^3,6

Monthly-plan essential labs are advertised from $89; larger panels and some states cost more. Shipping is charged per medication order and excluded from membership under the terms, with a waiver for product orders over $500. Obtain an itemized quote for your medication, concentration, quantity, supply duration, labs and shipping. Included TRT or B12 promotions are not a promise that tirzepatide is included.^3,5,6

The agreement requires three consecutive membership payments regardless of treatment utilization, then recurring month-to-month renewal. The agreement identifies Vita Bella Health, LLC as a management organization, not the treating medical practice, and says it does not employ the clinicians or dispense medication. It coordinates prescriptions with preferred pharmacies, but that general arrangement does not identify the facility assigned to a particular prescription. Obtain the treating practice, clinician, preparing pharmacy and dispensing pharmacy.⁵

Confirm the selected medication, prescribed preparation, location eligibility and accepted all-in quote before paying. A broad U.S. service claim does not establish availability for every treatment in every state.
