Four-week billing versus prepaid totals

The public pricing table lists $449 auto-billed every four weeks, $599 paid upfront for four weeks plus $35 shipping, $1,425 paid upfront for 12 weeks, and $4,788 paid upfront for 48 weeks. The subscription, 12-week and 48-week options advertise free shipping. The longer prepaid plans show averages of $475 and $399 per vial respectively; these are not month-to-month charges.⁴

The homepage’s starting-at-$399 headline should therefore not be read as a calendar-month subscription price. The named table ties $399 to the per-vial average of the 48-week prepaid plan. A public menu still does not establish a personal quote, prescribed dose or the final cost if additional medication is required.^3,4

The tirzepatide FAQ says additional medication for higher prescribed doses costs $150 per additional vial every four weeks. Its announcement and FAQ price the consultation at $1, while the named page also uses a “free consultation” link.
