FormidableMD Tirzepatide Review: Visit Fees & Supply Costs

FormidableMD advertises $199 for an initial consultation and $99 for a follow-up, without a monthly membership. Medication and laboratory charges are separate. Its tirzepatide table lists $199–$319 against different medication amounts but does not state the supply duration, so those figures should not be treated as monthly prices or weekly dosing instructions.^3,4

Build a cost estimate around visits and supply

The current table pairs 10 mg with $199, 20 mg with $239, 30 mg with $279 and 40 mg with $319. Ask what each purchase supplies, the prescribed concentration, number of administrations and refill timing. The listed amount alone does not establish the patient's weekly dose or how long a vial lasts.³

The billing agreement excludes medication, laboratory testing, medical supplies, equipment and nutrition coaching from the visit fee. An initial visit plus the lowest listed medication amount would be $398 by arithmetic, before any other necessary costs; it is not a confirmed first-month package.^3,4

Follow-up frequency affects the total

The medical-care page schedules monthly follow-ups for medication patients during the first three months. Once the dose is stable and titration is unnecessary, it describes visits every three months during the first year or until the goal is met, then recommends six-month check-ins. Medication, formulation, frequency or dose changes require reassessment at a follow-up visit.³

These are the practice's published expectations, not a personal schedule or promise of clinical results. Confirm whether an additional requested visit carries the same fee and how to contact the clinical team between appointments.

Branded and compounded pathways

FormidableMD describes both approved and compounded GLP-1 options and says it can pursue prior authorization for a recommended medication on the patient's insurance formulary. Approval, coverage and a specific prescription are not guaranteed. The practice attributes its compounded sourcing to 503A pharmacies but does not identify your particular dispenser in the checked medical-care text.³

Appointment cancellation is not a medication return

The March 26, 2026 billing agreement says the appointment is charged when confirmed. Canceling more than 48 hours before the appointment permits a full visit-fee refund; cancellation within that window incurs a $75 fee. The agreement explains that clinical review may begin before the visit.⁴

The general Terms of Sale separately describe purchases as final, subject to stated exceptions and applicable law. Neither clause establishes a blanket prescription-denial refund or a right to return dispensed medication. Ask which visit, pharmacy and laboratory rules apply before payment.^4,5
