Form Health Tirzepatide Review: Care Fees, Insurance & Terms

Form Health publishes a $299-per-month self-pay care plan, including clinician and Registered Dietitian appointments, messaging and platform access. Medication and lab work cost extra. Its other main route bills participating insurance, including Medicare, with patient costs depending on benefits. Neither route establishes a fixed monthly tirzepatide total.⁴

The distinction matters if you are comparing Form with a medication-inclusive subscription. Paying for Form’s care does not guarantee a Zepbound prescription, drug coverage or a particular pharmacy price. The current medication page names Zepbound as its tirzepatide option and says treatment depends on clinical judgment.⁵

What the care plan includes

The program describes monthly video visits with a clinician and Registered Dietitian, ongoing messaging, and help with nutrition, behavioral health and physical activity. Its homepage describes an American Board of Obesity Medicine clinician within a physician-led team. Those are the company’s descriptions of its care model, rather than independent verification of an individual practitioner’s credentials.^4,6

Enrollment criteria include being at least 18, a BMI of 30 or higher, or 27 or higher with weight-related health risks. Form also requires an ongoing primary-care relationship and a visit within the previous 12 months. It requests records and permission to coordinate with that clinician. Meeting these public criteria does not establish acceptance or medication suitability.⁴

Form says it only prescribes FDA-approved medications. Its named weight-loss list includes Zepbound alongside medications containing different active ingredients. This is a prescribing service, not evidence that Form itself manufactures or dispenses the drug. Separate care charges from medication coverage

The published $299 self-pay fee covers care and platform access, while medication and labs are extra. Do not treat that amount as a starter-dose drug quote or add an assumed brand price to produce a guaranteed total.⁴

For insured patients, Form says most major private insurers and Medicare cover its care-team visits, labs and medication costs, subject to benefits. That statement is not a guarantee that your plan covers Zepbound for your diagnosis, that every service is in network, or that your deductible has been met. Form offers a billing contact to check benefits; prescription coverage remains a separate decision.⁴

A manufacturer offer has its own eligibility and refill conditions and does not automatically remove Form’s care charges.

Renewal, cancellation and what remains payable

The FAQ says membership can be canceled at any time. The Terms specify automatic renewal unless canceled before the current subscription period ends, with the account closing at that period’s end. A separate payment-authorization provision asks for email notice at least 15 days before a scheduled payment to revoke automatic-payment authorization. These describe different actions; do not assume canceling a subscription erases already incurred charges.^4,7

The Terms also distinguish covered medical services from optional subscription features. They describe access to covered providers without a subscription fee by emailing Form, while excluding subscription features and a connected scale from that route. That provision is not a promise of free visits: insurance cost-sharing and other charges can still apply.⁷

A cancellation right does not establish a prorated refund or a refund whenever no medication is prescribed. Obtain the applicable care, pharmacy and subscription charges before paying. If leaving the program, coordinate follow-up and prescriptions with your clinicians rather than assuming account cancellation is a medical stop instruction.

Clinical evidence and service quality

Form’s homepage reports average weight loss across its own treatment pathways. Those company-reported outcomes are not an independent trial of Form’s service, nor a forecast of your result. The clinical team should explain monitoring and what happens if medication is unsuitable or unavailable.⁶ Review Zepbound side effects, the gallbladder risk, and the thyroid cancer warning with your clinician.

Judge the provider on price, oversight and disclosure; judge the molecule on its trials.
